For staff — connecting this page to Zoho CRM

Only this page uses the custom Zoho code now

The intake form moved to a Zoho Forms-hosted form (connected to CRM natively inside Zoho — no code involved there). This appointment page is the only one still posting through zoho-config.js / zoho-api-config.js, so that's the only place left to configure.

Web-to-Lead route (default, no backend)

Zoho CRM → Setup → Developer Space → Web Forms → Create Web Form → Leads module.
Copy the form's action URL and the hidden xnQsjsdp / xmIwtLD values into zoho-config.js.

API route (optional, needs the Azure Function)

Set ZOHO_SUBMISSION_MODE = 'api' in appointment.js once SubmitToZoho is deployed — see azure-function/README.md for the full OAuth setup.

Document uploads → OneDrive

The intake form's document upload (and its OneDrive backend, UploadToOneDrive) isn't wired into this appointment page. If you want documents collected here too, or want Zoho Forms' own file uploads forwarded to OneDrive, that would go through Zoho Flow (a separate automation inside Zoho) rather than this site's code — let us know if you'd like that set up.
